    • For First-Time Patients

    0.General information

    Located the General information desk on the ground floor near the main entrance.
    Ask at General information desk if you are unsure which department to consult,
    Week days 8:00~11:00

    Reception Time

    Please refer to the “Outpatient announcement”.

    1.Registration for the First Time

    Please fill out the “Registration Form” on the table.
    Please make sure all the information (name, date of birth, address, telephone number) which the black line is fill out.

    2.Reception place

    ①Please submit the “Registration Form”, “Health Insurance Card” and other certificates and “Referral” at the window No.1.
    ②Please be seated and wait until your name is called.
    ③Once the registration is completed, you will be given a patient registration card and a clinical record and guidance to today’s consultation.

    3.Department

    ①Please submit a patient registration card and a clinical record at the each section.
    ②You will be given a medical history questionnaire to fill out. Please fill this out and submit the form.
    ③Please be seated and wait until your name is called.


    There are the elevator and stairs near the window No.1.
    1F General Practice and Emergency, Orthopedics, Ophthalmology, Urology,
    Rehabilitation, Radiology
    2F Internal Medicine, Surgery, Oral and Maxillofacial Surgery,
    Plastic Surgery, Pediatrics, Pediatrics Surgery, Otorhinolaryngology, Gynecology
    Perinatal Medical Center
    4F Obstetrics, NICU neonatal intensive care unit

    4.Consultation

    ①When your name is called at the department, please enter the consultation room.
    ②Showing the patient registration card for identification of a person.
    ③When your consultation is completed, an accounting slip will be given to you.
    ④Please proceed to the cashier, window No.3.
    内科診療

    5.Cashier

    ①Please submit the complete file to the cashier (window No.3)
    You will be given an accounting ticket. The number on the ticket is to inform you when the payment is ready.
    ②Once the calculation is finished, the digital board will indicate that your bill is ready or the payment by the automatic payment machine.
    NoteWhen a receipt is issued, a reservation slip for your next consultation and medicine slip will be attached to it.

    6.Medicine

    ①Prescriptions for dispensing pharmacies will issued for your medication in most cases. Please change medicine slip to your prescriptions at the window No.7.
    ③Please pick up your medicine at dispensing pharmacy located outside the hospital within four days (including the day of issuance).

    １．Registration for a follow-up：Patients with an appointment

    Please go to the today’s consultation, and submit a Reservation Slip to the window.
    Your reservation slip for your next consultation will be attached to the receipt.

    Registration for a follow-up：Patients without an appointment

    Please register using the automatic follow-up registration machine, and go to the today’s consultation,

    Patients with an Examination reservation

    CT, MRI, Ultrasonography, Gastric camera, Colon camera

    Please go to the each examination reception desk prior to your reservation time.

    Please be seated and wait until your name is called.
